say, if someone has one doctor but fills two prescriptions at two different drugstores, does the doctor know that? Does the patient info go into one database or something?

depends on the drug and the state. Most states have a central drug database that tracks controlled substances (things like Vicodin, Percocet, Xanax, Lyrica, etc.), where they are filled, # they got, what doctor wrote the prescription, days supply, and date they got it filled. Most pharmacists/doctors don’t look someone up in the system unless they have reason to (they think the person is a drug-seeker).
