I can think of two that really stand out

Harrison Ford is my actor for both

In Return of the Jedi when he is to fall out of the carbonite, you see him really sick and distorded.
well turns out he was really sick (polio i think) and was ina wheelchair. he actually falls forward to the ground from the chair.

In Raiders of the Lost Ark when he is in the market area and the guy with the giant sword challenges him, original Ford was to pick up a sword and fight him.

However a large number of the cast were suffering from water bone (some disease) and he didnt feel up to hand to hand fighting so he pulls out his pistol instead.

Mark Hammil (luke skywalker) had a interesting one:

In empire Strikes Back he is attacked by a ice monster and left with scars on his face.

However originaly none of this was planned.

Several days before shooting began, Hammil was involved in a auto accident. The ice monster was added to explain the scars.
