who decides if a movie opens nationwide or opens with a limited release how can a movie expect to make any money with a limited release in which most people wont be able to see it?

They do a preview screening before a selected target audience before it’s released. If the target audience likes it then they will screen it everywhere. If it’s not that well received then it gets a limited release and then goes to dvd quicker(sometimes straight to dvd.)
Also, not being cynical, but some movies are made as a tax write off for the companys involved. It doesn’t matter if it’s not seen by a wide audience because it’s supposed to lose money.
